Please bring your medical card to the surgery if possible. All newly registered patients will be asked to complete a simple questionnaire and invited to attend for a new patient examination with one of the practice nurses. Four of our doctors currently take on new patients on a rota basis but a special request to go on the list of a particular doctor will always be considered. Dr Louisa Campbell is happy to see any patient , but all patients are registered with one of the partners. Print off the registration form below and email/fax or hand it into us: |
